29 CFR 1910.37(a)(4): Safeguard(s) designed to protect employees during an emergency (e.g., sprinkler systems, alarm systems, fire doors, exit lighting), were not in proper working order at all times:  Employees in the warehouse are exposed to burn and smoke inhalation injures and the emergency "Exit" illuminated signage was not operational in the following areas of the Grandview Outlet:  a) East wall, north end of the store;  b) North wall of Dock Room;  c) North wall of Back Room where the trash is stored.

29 CFR 1910.305(b)(2)(i): Pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose:  Employees were exposed to electrical shock and burn hazards and junction boxes were not provided with suitable covers in the following areas:  a) East wall of the Grandview Outlet Back Room where the trash is stored.  b) East side at the cash register of the Grandview Outlet store (electrical feed for security camera monitor).  c) Outside the Grandview Outlet Store on a concrete ledge South of the entry doors; southern most receptacle in the series of outdoor use receptacles.  The Grandview Buildings & Supply, Inc, was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1090066, citation number 1, item number 10 and was affirmed as a final order on December 29, 2015, with respect to a workplace located at 512 N. Webster, Taylorville, IL 62568.

29 CFR 1910.303(b)(1)(vii): Safety of equipment was not determined using  considerations for classification by type, size, voltage, current capacity, or specific use.  Employees in the Home Center were exposed to minor electrical shock from a series of 120V/20A circuit breakers (labeled 1 through 10 in Panel A) that were used to switch the door lighting circuits daily and the circuit breakers were not rated for switch duty.
