On or about February 11, 2016, in the maintenance shop area, the employer allowed the employees to operate a DeWalt Table Grinder (Model#: DW758, Serial#: 200406-YL0170), that did not have a work rest nor a tongue guard attached. a. In the maintenance shop, one (1) DeWalt Table Grinder ( Model#: DW758, Serial#: 200406-YL0170), did not have a work rest attached. b. In the maintenance shop, one (1) DeWalt Table Grinder ( Model#: DW758, Serial#: 200406-YL0170), did not have an adjustable tongue attached.
