Pickling Bay and Galvanizing Area: On or about February 1, 2015, employees were exposed to a struck by or crushing hazard when operating an Yale 6 ton overhead bridge crane (serial no. unclear) with multiple hoist that did not have a locking latch on the crane hooks on west hoist #2 and east hoist #2.
