29 CFR 1918.35: Employees were working around open weather deck hatches which were not protected by coamings to a height of 24 inches (0.61 m), or by taut lines or barricades at a height of 36 to 42 inches (0.91 m to 1.07 m) above the deck. F/V Blue Pacific, Weather deck: On or about 9 February 2016 and times prior thereto, The Hydraulic Room hatch, measuring 18.5" x 24", was open with a coaming of approximately 3 inches. This condition exposed employees to a foot fall hazard.

F/V Blue Pacific, Factory area: On or about 9 February 2016 and times prior thereto, the yellow synthetic rope in use for positioning the freezer hatch exhibited excessive wear. This condition exposed employees to a struck by hazard.

This condition exposed employees to a caught in hazard.
