29 CFR 1915.56(b)(2): Welding cable was not free from repair or splices for a minimum distance of 10 feet from the cable end to which the electrode holder was connected. P/V Akutan, factory area; On or about 8 February 2016 and times prior thereto, employee was performing welding operations with a welding stinger with damage 60" back from the stinger. Damaged cord was taped with black tape. This condition exposed employees to an electrical shock and burn hazard.

This condition exposed employees to amputation hazard.

This condition exposed employees to a 166" fall hazard.
