29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE):  Factory Deck:  On or about February 08, 2016 and at times prior thereto, where the caustic cleaning product SR100 was  used to facilitate cleaning and sanitizing of the processing equipment, the employer had not assessed the work area for hazards that were or likely to be present.  This condition exposed the employee to skin and injuries from contact with the caustic liquid.

29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification which included the requirements as outlined in 29 CFR 1910.132(d)(2):   Factory Deck:  On or about February 08, 2016 and at times prior thereto, where the caustic cleaning product SR100 was  used to facilitate cleaning and sanitizing of the processing equipment, the employer had not assessed the work area and provided a written certification that the assessment had been done.  This condition exposed the employee to skin and injuries from contact with the caustic liquid.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:  P/V Akutan:  On or about February 08, 2016 and at times prior thereto, where employee were exposed to hazardous chemicals including caustic cleaning agents, petroleum, oil and lubricant products, refrigerant gases and liquids, and welding electrodes; the employer had not developed a written hazard communication program.  This condition exposed employees to eye, skin, inhalation, and ingestion hazards from lack of chemical hazard information.
