29 CFR 1910.215(b)(5): The angular exposure of grinding wheel periphery and sides for safety guards used on cutting-off machine(s) employing the wheel periphery exceeded 150 degrees: a) Cut-off Saw Department - On or before February 11, 2016 an abrasive cut-off saw had an angular exposure of the grinding wheel periphery in excess of 190 degrees when in the operating position. An employee suffered an amputation injury operating this saw when his finger contacted with the blade. b) Cut-off Saw Department - On or before February 11, 2016 a second abrasive cut-off saw had an angular exposure of the grinding wheel periphery in excess of 190 degrees when in the operating position.

29 CFR 1910.215(b)(10)(v): For cutting-off wheels larger than 16 inches diameter and where speed does not exceed 14,200 surface feet per minute, safety guards as specified in Figures O-27 and O-28, and in Table O-1 were not used: a) Cut-off Saw Department - On or before February 11, 2016 an abrasive cut-off saw with a blade of approximately 20 inches and an approximate surface speed of 12,281 feet per minute had a 1/16 inch guard thickness which was less than the 3/32 minimum required specified in table O-1. b) Cut-off Saw Department - On or before February 11, 2016 a second abrasive cut-off saw with a blade of approximately 20 inches and an approximate surface speed of 12,281 feet per minute had a 1/16 inch guard thickness which was less than the 3/32 minimum required specified in table O-1.
