29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:      On or about February 16, 2016, at the trenching site on the corner of Kent Dr. and May Ave. in Oklahoma City, OK, an employee was allowed to work in a trench more than 4 feet deep that had no ladder or other safe means access/egress within 25 feet of the employee.  Employees were exposed to the hazard of struck by falling soil.

29 CFR 1926.651(j)(2): Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary:      On or about February 16, 2016, at the trenching site on the corner of Kent Dr. and May Ave. in Oklahoma City, OK, employees were allowed to work in a trench with spoil piles within 2feet of the trench edge, exposing employees to the hazard of being crushed by loose dirt or soil.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c).       On or about February 16, 2016, at the trenching site located on the corner of Kent Dr. and May Ave. in Oklahoma City, OK, no protective devices were in place to protect an employee working in a trench that was more than 5 feet in depth, exposing employees to the hazard of being crushed by fall soil.          Southwest Waterworks was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.652(a)(1), which was contained in OSHA inspection number 893402, citation number 1, item number 2, and was affirmed as a final order on May 16, 2013, with a final abatement date of April 25, 2013, with respect to a workplace hazard located at N.W. 150th and May Oklahoma City, OK 73134.
