29 CFR 1904.39(a)(2): 29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four hours. a) Advanced Drainage Systems, Inc. dba Green Line Polymers / 22793 Route 66, Shippenville, PA: On or about February 17 and times prior thereto, the company did not report a work related amputation within 24 hours of it occurring. The amputation occurred on or about January 31, 2015 at 7:00 PM. The amputation was not reported to OSHA until February 12 at approximately 2:00 PM.
