Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were operated a powered industrial truck without a seat belt installed.    On or about 2/18/2016 at 293 Harrisburg Road SW, Milledgeville, GA: Where employees operated the Caterpillar powered industrial truck to lift and move material equipment that was not equipped with a seat belt.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:Note:  Toeboards are only required when persons can pass, there is moving equipment or there is equipment with which falling materials could create a hazard.  On or about 2/18/2016 at the stacker equipment located at 293 Harrisburg Road SW, Milledgeville, GA: The employees working on the platform without a standard guardrail system were not protected from a fall hazard at about 7 feet 11 inches from the ground level.

29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D):  On or about 2/18/2016 at the Debarker equipment located at 293 Harrisburg Road SW, Milledgeville, GA:  Employees performing service and maintenance such as but not limited to unjamming, repairs, setups, and maintenance on the Debarker, Stick Stacker, and other machines and equipment were not provided specific written energy control procedures for locking and tagging out all of the energy sources.  Energy sources such as but not limited to electrical, mechanical, pneumatic, and hydraulic.  On or about 2/11/2016 at the Debarker equipment one of the two employee was injured while attempting to unjam a tree log from the debarker equipment.
