29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level was protected from falling by a guardrail system, safety net system, travel restraint system, or personal fall arrest system. (a) On the catwalk at the Green Chain conveyor machine: On or about February 5, 2024, and at times prior, employees were exposed to a fall hazard of approximately twelve (12) feet when the metal panels did not extend the length of the catwalk near the front side of the conveyor belt of the Green Chain machine.
