29 CFR 1926.501(b)(11): "Steep roofs." Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.  a. Located at 6505-6514 Yacca Way in Canal Winchester, Ohio: On or about February 22, 2016, employees were installing wooden trusses and sheathing on the roof of a three story multi-family apartment building without the use of fall protection, exposing employees to a fall hazard of approximately thirty-one feet.

29 CFR 1926.501(b)(1): "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a. Located at 6505-6514 Yacca Way in Canal Winchester, Ohio: On or about February 22, 2016, employees were performing framing work from the top plate of the second story wall of the building without the use of fall protection, exposing the employees to an approximately nineteen foot fall hazard.   Martin Venegas was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 314593336, citation number 1, item number 2, and was affirmed as a final order on June 24, 2011,with respect to a workplace located at Parkway Crossing Apartments, Epic Way, Grove City, Ohio 43204.

On or about February 22, 2016 and at times prior, the employer employed approximately sixteen employees and did not keep OSHA injury and illness records for the calendar years 2015 and 2016.
