29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a) 3901 Michael Blvd, Mobile, AL: on or about January 30, 2016 the employer failed to ensure each employee exposed to fall hazards while sheeting a roof received training on the use of fall protection and procedures to be followed to minimize the hazards.

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours.    a) 3901 Michael Blvd, Mobile AL 36609; On or about January 30, 2016 the employer had an employee fall from a roof and was hospitalized. The employer failed to report the work-related hospitalization to OSHA within twenty-four (24) hours.
