Embassy Powdered Metals Inc., - On or about 2/18/16, and at times prior thereto, employees performed servicing and maintenance work on machines and equipment that include, but were not limited to, sizing presses and sintering furnaces. A lockout/tagout program consisting of procedures, employee training, and periodic inspections was not developed, documented, and implemented.
