29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):  a) At the job site, at the west side of the house on the second floor, on or about and at times prior to February 17, 2016, the employer did not ensure a wood fabricated scaffolding system that measured approximately ten feet and three inches in height from the second floor and twenty three feet in height from the ground level, was fully planked, exposing employees to fall hazards.

29 CFR 1926.451(e)(1): Portable ladder, hook-on ladder, attachable ladder, stair tower, stairway-type ladder, ramp, runway, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used to access scaffold platform more than 2 feet above or below a point of access:  a) At the north side of the house, on the second floor, on or about and at times prior to February 17, 2016, the employer did not ensure a proper means of access to the working platform of a wood fabricated scaffolding system, that created a fall distance of ten feet and three inches to the floor level, exposing employees to fall hazards.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.  a) At the west side of the framed house and on the second floor, on or about and at times prior to February 17, 2016, the employer did not ensure employees who were performing framing work from the working platform of a wood fabricated scaffold system, with a fall distance of twenty three feet to the ground level and ten feet three inches to the second floor, were protected from falling, exposing the employees to the hazards associated with death, paralysis, broken bones, sprains, lacerations, and bruises.
