29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: a) On or about February 23, 2016 and times prior; in trimmer feed table: The employer failed to ensure operators were properly trained and authorized to lock out equipment while clearing jams. Operators clearing jams only turned off the electrical power by pushing the e-stop for the motors for the rollers and chains instead of turning off the disconnect and locking it out.
