29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace:    a) On February 22, 2016, Handi Products, Inc. dba Handi-Ramp, Inc., Libertyville, IL did not ensure that all their employees, including the spray painter(s) that wear respiratory protection when performing spray painting operations, such as the MSA Advantage 200 LS half-mask respirator equipped with organic vapor cartridges and overspray filters, were provided medical evaluations.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.134(f)(2): The employer did not ensure that employees using a tight-fitting facepiece respirator were fit tested prior to initial use of the respirator, whenever a different respirator facepiece (size, style, model or make) is used, and at least annually thereafter:    a) On February 22, 2016, Handi Products, Inc. dba Handi-Ramp, Inc., Libertyville, IL did not ensure that all their employees, including the spray painter(s) that wear respiratory protection when performing spray painting operations, such as the MSA Advantage 200 LS half-mask respirator equipped with organic vapor cartridges and overspray filters, were provided annual fit tests prior to initial use and at least annually thereafter.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.134(k)(6): The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer:  a) On February 22, 2016, Handi Products, Inc. dba Handi-Ramp, Inc., Libertyville, IL, did not provide employees, such as the MIG welders who voluntarily wear 3M brand, model 8574, N95-rated filtering facepiece respirators, the information contained in Appendix D to the Respiratory Protection Standard, 29 CFR 1910.134.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
