29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder. On or about February 23, 2016 employees used an extension ladder to gain access to a roof where the ladders length did not allow three feet of extension past the roof edge, exposing the employee to a fall of over 25 feet. Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about February 23, 2016 employees were not protected from a fall while performing residential roofing activities, exposing employees to a fall of over 25 feet. INFINITY REMODELING INC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(11), which was contained in OSHA inspection number 108206, citation number 1, item number 1, and was affirmed as a final order on March 13, 2012, with respect to a workplace located at 17500 S. 84th Ave. Tinley Park, IL 60487. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
