29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds shall be fully planked or decked between the front uprights and the guardrail supports. 199 E Houston St, NY, NY: On or about 2/22/2016, two Titan Industrial Services Corp. employees were working from a welded tubular frame scaffold that was not fully planked. The employees were exposed to fall hazards between 12 and 18 feet.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level. 199 E Houston St, NY, NY: On or about 2/22/2016, two Titan Industrial Services Corp. employees were working from a welded tubular frame scaffold. The scaffold was not equipped with guardrails and the employees were not using any other form of fall protection. The employees were exposed to fall hazards between 12 and 18 feet.
