29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a.) AK Tube, LLC. located at 30400 E. Broadway Walbridge, OH; On February 11, 2016, the employer did not assure employees were protected while using the A1 Mill Hydraulic Test Press. An employee was holding a tube on the cone test fixture with the left hand and activated the hydraulic press with the right hand, resulting in an amputation of the tip of the left index finger.
