Broadway, Walbridge, Ohio, 43567: On or about January 4, 2021, the employer did not assure employees were protected from entering the point of operation while using Enerpac Hydraulic Press that resulted in an amputation of the tip of the left index finger.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure: A) AK Tube, worksite located 30400 E. Broadway, Walbridge, Ohio, 43567: On or about January 6, 2021: In the Toolng Department, the employer used a temporary power cord as a substitute for fixed wiring in that the flexible cord was used on a continual basis to power a Enerpac Hydraulic Press there by exposing employees to an electrical hazard.
