29 CFR 1910.22(a)(3): Every floor, working area, or passageway was not kept free from protruding nails, splinters, holes, or loose boards:    On or about February 24, 2016, and times prior thereto, at the non-ferrous picking station, the platform over 4 foot high did not have an adequate flooring such that the flooring made of expanded metal was deteriorated and contained damaged/loose metal, which compromised the integrity of the floor.

29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to Items of this sec(a).    On or about February 24, 2016, and times prior thereto, at the Non-Ferrous Picking Station, employees were exposed to amputation hazards when performing belt adjustments to the recovery conveyor tail pulley. The employer did not establish a procedure to control and isolate hazardous energy when servicing the conveyor belt machine.
