29 CFR 1910.147(c)(6)(i): The employer did not conduct an annual or more frequent inspection of the energy control procedure to ensure that the procedure and requirements of this standard were followed: On March 13, 2017, and times prior thereto, at the Non-Ferrous Picking Station, employees were exposed to amputation hazards when performing belt adjustments to the recovery conveyor tail pulley. The employer did not conduct inspections of the energy control procedures to ensure isolation of hazardous energy when servicing the conveyor belt machine.

29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to Items of this sec(a). On or about March 13, 2016, and times prior thereto, at the Non-Ferrous Picking Station, employees were exposed to amputation hazards when performing maintenance and or belt adjustments to the recovery conveyor tail pulley. The employer did not establish a procedure to control and isolate hazardous energy when servicing the conveyor belt machine. All Valley Metal Recycling, LLC, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(c)(4)(ii), which was contained in OSHA inspection number 1127584, citation number 1, item number 4 and was affirmed as a final order on September 6, 2016 in respect to a workplace located at 16100 N. Val Verde Road, Donna, Texas 78537.
