29 CFR 1915.56(b)(4): When welding cable became worn to the extent of exposing bare conductors, the portion exposed was not protected by means of rubber tape, friction tape, or other equivalent insulation.  (a)  Welding shop:  On or about February 22, and times prior thereto, a welding cable was cut to the conductor.  This exposed employees to potential shock hazard if they came in contact with the conductor while welding.
