29 CFR 1910.269(j)(2)(iii): Live-line tools used for primary employee protection shall be removed from service every 2 years, and whenever required under paragraph (j)(2)(ii) of this section, for examination, cleaning, repair, and testing as follows: Employee(s) using Hubbell Chance hot sticks dated October 3, 2013, Epoxiglas Hot Sticks dated June 30, 2011, and a Hastings hot stick dated March 2012 as tools for energized electrical equipment were exposed to electrical hazards due to no examination / testing being conducted every 2 years on the hot sticks.

29 CFR 1910.303(b)(1)(i): Electric equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees. In that, the employer's failure to consider the suitability of energized electrical equipment being exposed to water: Water leaking from a plugged condensate pan for the Dry End HNV system would flow down the east wall of the Casting Department impacting two electrical panel boxes and a disconnect box. The water flowing from the wall and energized electrical boxes onto the floor exposed employee(s) in the department to electrical hazards.
