29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been  taken out of service until restored to safe operating condition(s):   a. On or about February 25, 2016, at the fabrication shop, the Caterpillar 60 forklift being used at the site to unload granite slabs and move them inside the shop was defective and unsafe in that the backup alarm was not operational, there was an hydraulic leak on the mast and the right rear wheel was worn out and did not balance evenly with the new left rear wheel, exposing employees to struck-by hazards.
