On or about March 10, 2016, an employee was required to wear a N95 dust respirator when dry cutting a kitchen sink template in granite countertop and the employer did not establish and implement a written respiratory protection program that addressed the worksite-specific procedures such as but not limited to employee medical evaluations, fit testing, and training on the use, maintenance and care of the respirators.
