29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system: a. While conducting roofing replacement operations on a second story commercial building flat roof, on or about February 25, 2016, five roofing employees, not protected by any form of fall protection systems, and are exposed to a 29-foot, 6-inch fall to the ground level parking lot and driveway below. b. While conducting roofing replacement operations on a second story commercial building flat roof, on or about February 25, 2016, three roofing employees, not protected by any form of fall protection systems, and are exposed to a 13-foot fall to the lower level of the first story roof below. The employer fails to ensure effective protection against falls as required by the standard, as demonstrated on February 25, 2016, when employees installing a multi-ply commercial roofing system, on the second story flat roof of a commercial medical facility, located at 1325 San Marco Boulevard in Jacksonville, Florida, were exposed to 13 and 29-foot, 6-inch fall hazards above lower levels in that they did not have any form of fall protection systems in place at the time of the hazard exposure. Dibble Roofing Co., Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 1926.501(b)(13), which was contained in OSHA inspection number315485417, citation number 1, item number 1 and was affirmed as a final order on June 19, 2012, with respect to a workplace located at 3853 Millpoint Drive, Jacksonville, Florida 32257.

29 CFR 1926.502(f)(2): Warning line systems did not consist of ropes, wires, or chains, and supporting stanchions erected in accordance with (f)(2)(i) through (f)(2)(v) of this section: a. While conducting roofing replacement operations on a second story commercial building flat roof, on or about February 25, 2016, eight roofing employees were exposed to fall hazards ranging from thirteen to twenty-nine feet, six inches as the Warning Line system in place was constructed of plastic caution tape in lieu of the required rope, wire, or chain.
