29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  (a) Front of building near exit door:  A relocatable power tap was used to power a microwave and a refrigerator which is not in accordance with the listing and labeling and exposes the employees to shock and burn hazards.  (b) Inside wall near oil tank: A relocatable power tap was used to power battery chargers, shop lights, and other tools and equipment in an area where dust from sanding vehicles covered the tap which is not in accordance with the listing and labeling and exposes the employees to shock and burn hazards.

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  (a) Employees were constructing an office space at the rear of the facility where the employee working on the ground floor was not required to wear a protective helmet exposing him to struck-by hazards from falling objects.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  Rear of facility:  An employee was working above the ceiling trusses without the use of fall protection exposing him to a fall of approximately ten (10) feet to the concrete floor below.
