BriMark Builders, LLC, 4995 Station Road, Erie, PA 16510, southeast side of the structure, on or about February 26, 2016 and times prior thereto: Vehicles and equipment including, but not limited to, rough terrain forklift trucks and drywall trucks drive within three feet of a 1000 gallon LPG tank. The LPG tank was not properly protected against potential vehicular damage.

29 CFR 1926.405(a)(2)(ii)(I): Flexible cords and cables used for temporary wiring were not protected from damage: A.) BriMark Builders, LLC, 4995 Station Road, Erie, PA 16510, northeast side of the building, on or about February 26, 2016 and times prior thereto: A generator was providing electrical power to flexible electrical extension cords. The flexible electrical extension cords were powering equipment including, but not limited to, the InsulMax Blow-In Insulation Machine. The flexible electrical extension cords were running under a sliding wooden door. The cords were not protected against physical damage created by the siding door. B.) BriMark Builders, LLC, 4995 Station Road, Erie, PA 16510, northeast side of the building, on or about February 26, 2016 and times prior thereto: A flexible electrical extension cord was powering equipment including, but not limited to, a Makita Circular Saw (M/N: 5007NB). The flexible electrical extension cord was running under a sliding wooden door. The cord was not protected against physical damage created by the siding door.
