29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated, the person certifying that the evaluation has been performed, the date(s) of the hazard assessment, and, which identifies the document as a certification of hazard assessment: a.) Facility - On or about 2/29/16, on a daily basis employees use pressure washers to rinse vehicle exteriors; change out chemicals drums (corrosives); pump chemicals from one barrel into another barrel (corrosives); have exposure to wet walking/working surfaces; and, have exposure to noise from car wash equipment. The employer has and provides a variety of personal protective equipment including safety glasses; goggles; nitrile gloves; heavy rubber coated gloves; ear plugs; coats; boots; etc. The employer had not verify that the required workplace hazard assessment had been performed through written certification that identified the workplace evaluated; the person certifying the evaluation and the dates of the hazard assessment. ABATEMENT CERTIFICATION REQUIRED

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: a) Facility - On or about 2/29/16, employees use corrosive chemicals such as Simoniz Dust Buster (corrosive); Simoniz Magic Lustre Summer Breeze (corrosive) for car washing. Employees must remove the hoses from the barrels of chemicals to change out the hoses when empty and have exposure to corrosive chemicals. The eyewash is located in the employee rest room, through two doors; the eyewash water must be activated with 2 hands (hot and cold water faucets) and then a knob pulled; the water is not tepid (60-100 degree F), rather cold; and, water pressure is not consistent. The eyewash station does not meet the requirements of ANZI A358 for no obstructions (doors); tepid water; and, activation in 1 second or less. The employer does not have suitable facilities for quick drenching or flushing of the eyes in the immediate work area for emergency use. ABATEMENT CERTIFICATION REQUIRED
