29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: a) 4525 Walnut St, Philadelphia, PA - The employees on the walking/working surface were not protected from falling through a floor hole (access hatch) to the next level by use of some type of fall protection system thus exposing the employees to a fall hazard. This condition was observed on or about February 26, 2016. Masterkey Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard or essentially similar standard, 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1068678, citation number 2, item number 1 and was affirmed as a final order on November 12, 2015, with respect to a workplace located at 113 Walnut St, Philadelphia, PA. Masterkey Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard or essentially similar standard, 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 960212, citation number 1, item number 1 and was affirmed as a final order on April 28, 2014, with respect to a workplace located at 1701 South Street, Philadelphia, PA.
