29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator before the employee was fit tested or required to use the respirator in the workplace: a) At the facility, on or about March 2, 2016 - An employee was not provided a medical evaluation prior to donning a Clemco Supplied-Air Respirator. An employee is required to wear a Clemco Supplied-Air Respirator during abrasive blasting operations.

29 CFR 1910.1200(h)(3)(iv): The details of the hazard communication program developed by the employer, did not include an explanation of the labels received on shipped containers and the workplace labeling system used by their employer; the safety data sheet, including the order of information and how employee could obtain and use the appropriate hazard information: a) At the facility, on or about March 2, 2016 - Employees were not provided training the new labeling elements and the SDS format. Employees are exposed to Chromium (VI) when welding on stainless steel.
