29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated, the person certifying that the evaluation has been performed, the date(s) of the hazard assessment, and, which identifies the document as a certification of hazard assessment: (a) On or about February 20, 2016, at worksite 4515 E 75 Terr, Kansas City, Missouri.- employees were exposed to amputation and laceration hazards in that a written hazard assessment certification had not been completed to certify the PPE needs and requirements for the employees operating the VA 0050 stacker machines. Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.
