29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: JOBSITE: The employer did not ensure through providing specific instructions, trainings and visual observation that the employee, required to cut the ductile iron pipe in the bottom of the trench with a Stihl Cut0off saw was operating the saw safely at all times by recognizing and avoidance of unsafe practices. On or about 02/29/2016, the above saw kicked back and hit the employee's face while the employee was performing the above task, and the employee sustained serious injuries from the incident.
