29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered.    On 3/3/16, at the Watkinsville Georgia facility, in the Clean Sort area, employees were exposed to head and face injuries as the blunt end of the clothes rack was not protected and the employer did not provide protective equipment so as to prevent employees from hitting their face or head on the rack.

29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment.  On 3/3/16, at the Watkinsville, Georgia facility, the employer did not have each authorized employee performing service and maintenance to equipment affix their own lockout lock to the equipment on which work was being performed. i.e. Authorized employees with locks were locking out for other authorized employees who were not provided locks by the employer.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips or sparks.  On 3/3/16, at the Watkinsville Georgia facility, in the Flat Iron area, employees were exposed to nip points as geared rollers an conveyor systems on the Flat Iron # 3 machine (aka 6 Roll Super Sylon machine) were not guarded. Areas included: 1 - at the side of the machine next to the aisle - reach to hazard area is 9 inches and 3 feet from floor levels 2 - at the front of the machine at the infeed side -- reach in to conveyor rollers is 4 inches
