29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets:  a. On or about February 29, 2016, at the back of home, the employee installing plywood underneath other employees working on the roof was not wearing a hard hat and was being exposed to head injuries.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):     a. Employees performing decking at elevations up to 25 feet or more above the ground are not protected from fall hazards by a fall protection system.    The employer fails to assure effective protection against falls as required by the standard, as demonstrated on February 29, 2016, when employees installing truss bracing and decking on a 6:12 pitch residential home located at 4904 SW 58th Place, Ocala, FL are exposed up to a 25 feet fall hazards in that they are not protected by a fall protection system.     Garcia Carpentry, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1014642 citation number 01, item number 001 and was affirmed as a final order on February 12, 2015, with respect to a workplace located at 1645 Spinfish Drive, Apopka, FL 32712.

On or about February 29, 2016, second floor, employees were using the top step of a step ladder to gain access and exit from the trusses and were exposed to an 8 feet fall hazard.     Garcia Carpentry, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(13), which was contained in OSHA inspection number 1014642, citation number 01, item number 002 and was affirmed as a final order on February 12, 2015, with respect to a workplace located at 1645 Spinfish Drive, Apopka, FL 32712.
