29 CFR 1926.404(b)(1)(iii)(C): The employer did not ensure that each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, were visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage. (a) On or about March 2, 2016, on Level C1 at the Hollywood Casino worksite, employee(s) were utilizing an HDX task light that was missing a grounding pin while installing wallpaper. Employees were exposed to electrical hazards.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: (a) On or about March 2, 2016, on Level C1 at the Hollywood Casino worksite, the extension cord powering the HDX task light did not have strain relief in that the sheathing was pulled back from the plug end. Employees were exposed to electrical hazards.
