29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: a. 3818 Braselton Hwy., Dacula, GA (Hamilton Mill Animal Hospital) - Employee were exposed to fall hazard. An employee was not utilizing the a means of fall protection while working and walking approximately 14 feet and greater above ground level, on the elevated roof with a slope pitch of 4-10:12 on various areas of the roof, installing felt paper on the newly constructed commercial building, on or about 03/04/16.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access: a. 3818 Braselton Hwy., Dacula, GA (Hamilton Mill Animal Hospital) - Employee were exposed to fall hazard. An employee was using a 24ft. portable extension ladders that did not extend 3 feet above the upper landing while accessing the ladder to get to the roof of the commercial building, on or 03/04/16.
