29 CFR 1910.147(c)(4)(i): Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section. a. Production area - On or about December 04, 2015, procedures were not developed, documented, and utilized for the control of potentially hazardous energy by employees required to service the Tipper Tie Machine. Employees were thereby exposed to the hazards associated with the unexpected re-energization of machinery capable of causing amputation. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1910.147(d)(6): Verification of isolation. Prior to starting work on machines or equipment that have been locked out or tagged out, the authorized employee shall verify that isolation and deenergization of the machine or equipment have been accomplished. a. Production area - On or about December 04, 2015, employees required to regularly maintain the Tipper Tie Machine, were not required to release stored / potentially hazardous energy prior to starting work. Employees were thereby exposed to the hazards associated with the unexpected re-energization of machinery capable of causing amputation. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).
