29 CFR 1910.132(f)(1)(iii): Employee(s) required to use PPE by this section were not trained to know how to don, doff, adjust, and wear PPE: (a) Secure chemical storage area - On or about April 8, 2025, the employer did not ensure that employees wore PPE appropriately when handling hazardous cleaning chemicals. While dispensing Zee Company GS-153 (contains potassium hydroxide and sodium hypochlorite), an employee incidentally splashed the chemical on their pantleg which was worn tucked into their chemical resistant boot. The chemical contacted their leg inside the boot resulting in a chemical burn that required hospitalization of the affected employee, debridement of the wound area, and skin graft. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: (a) Secure chemical storage area - On or about April 8, 2025, the employer did not ensure that an employee was provided effective information and training on hazardous chemicals prior to being assigned to work with Zee Company GS-153 (contains potassium hydroxide & sodium hypochlorite). The employee did not immediately flush GS-153 from their skin after making incidental contact resulting in a chemical burn that required hospitalization of the affected employee, debridement of the wound area, and skin graft. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
