29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a.   Employees installing framing and sheathing at elevations of six feet or more above the ground are not being protected from fall hazards by a fall protection system:  The employer fails to assure effective protection against falls as required by the standard, as demonstrated on March 4, 2016, when roofing employees installing Boral Tile Seal underlayment on the 6:12 pitch front slope of the San Salito Community Amenities Center at 445 Pullman Circle in Saint Augustine Florida were exposed to a 10 feet fall hazard above the ground in that they did not have a fall protection system installed.  The employer, K. & G. Construction Co., Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1926.501(b)(13), which was contained in OSHA inspection number 1080845, citation number 1, item number 1 and was affirmed as a final order on September 16, 2015, with respect to a workplace located at 6977 San Jose Blvd, Jacksonville, Florida.
