29 CFR 1926.1051(a): Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided: a) Construction site, East side of site - On or about March 8, 2016, The employer did not provide a safe means of access for employees required to work inside an enclosed foundation area while completing reinforcing bar construction. Employees accessed the work area by climbing the reinforcing bar wall, a distance of approximately 20 feet.
