On or about March 9, 2016, the employer did not ensure that employees operating the plasma cutting table were using safety glasses with filter lens with a minimum protective shade number of 8 to protect against radiant energy and flying sparks.    b.) On or about March 9, 2016, the employer did not ensure that employees tacking and welding were using a welding helmet.

29 CFR 1910.1200(h)(3)(iv):   The details of the hazard communication program developed by the employer, did not include an explanation of the labels received on shipped containers and the workplace labeling system used by their employer; the safety data sheet, including the order of information and how employee could obtain and use the appropriate hazard information:    On or about March 9, 2016, the employer did not provide training or information to employees working with hazardous chemicals on the details of the new hazard communication standard which encompasses the Global Harmonization System(GHS).
