29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:   On or about March 7, 2016, at a worksite located at 3921 Medina Road, Akron, OH 44333, during construction activities including, but not limited to, roofing activities on a low slope roof, employees were exposed to eye hazards during nailing, cutting and weatherproof material installation activities.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:  On or about March 7, 2016, at a worksite located at 3921 Medina Road, Akron, OH 44333, during construction activities including, but not limited to, roofing activities on a low slope roof, employees were exposed to fall hazards approximatley 14 feet.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:  On or about March 7, 2016, at a worksite located at 3921 Medina Road, Akron, OH 44333, during construction activities including, but not limited to, roofing activities on a low slope roof, employees were exposed to fall hazards while mounting and dismounting the ladder.
