29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a) Cutting Room-An employee testing the five (5) inch pizza die press reached into the point of operation to remove foreign debris off the conveyor and amputated the tip of the right ring finger when the machine cycled, on or about February 4th, 2016. No abatement certification or documentation required.
