29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours: a. On or about March 4, 2016, at approximately 3:45pm, an accident occurred at the jobsite resulting in an amputation and hospitalization. The incident was reported late to OSHA March 8, 2016 at approximately 3:00pm, thereby the 24 hour duration period for reporting had expired.
