There was an accumulation of class II dust  approximately 10 inches high by the door and top of the electric cabinet located at the southeast corner of the work floor.  2. There was an accumulation of combustible dust approximately 4 inches high on the Previero Plastic & Rubber machine.   3. There was an accumulation of combustible dust mixed with oil on the extruder #2 that could heat up and ignite.
