29 CFR 1910.303(b)(1)(ii): Electrical equipment was not free from recognized hazards, based on the mechanical strength and durability, including, for parts designed to enclose and protect other equipment, the adequacy of the protection thus provided: On or about March 7, 2016, the employer exposed employees to electrical hazards from damaged cords found on: a. The cord for a box fan in the regrind shredder #3 north of the machine workroom. b. The miter cut off circular saw near the plug of the electric cord in the center of the machine workroom. Greenland Composites, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.303(b)(1)(ii), which was contained in OSHA inspection number 1131364, citation number 1, item number 8a and was affirmed as a final order on July 18, 2016, with respect to a workplace located at 10175 Webb Way, Fayetteville, AR 72702.

29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed: On or about December 7, 2016 and there prior, the employer exposed employees to electrical hazards from: a. An unused opening on the bag massage electric switch box located on the north side of the machine workroom. b. An unused opening on the Weight System conduit box located on the northeast side of the machine workroom. c. The covers of the receptacles for the miter cut off saw in the center of the machine workroom that was missing screws. Greenland Composites, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1910.305(b)(1)(ii)), which was contained in OSHA inspection number 1131364, citation number 1, item number 10b and was affirmed as a final order on July 18, 2016, with respect to a workplace located at 10175 Webb Way, Fayetteville, AR 72702.

29 CFR 1910.22(d)(1): On every building or other structure, or part thereof, used for mercantile, business, industrial or storage purposes, the loads approved by the building official were not marked on plates of approved design and securely affixed in a conspicuous place in each space to which they relate. On or about December 7, 2016 and there prior, the employer failed to mark the approved load limits for the overhead storage on the break room, exposing employees to struck-by hazards.
