29 CFR 1910.178(l)(2)(ii): The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace: a) On or about 3/1/16 at a Condominium Construction site at 767 Legacy Dr., Lewiston, NY: An employee was operating a Lull 1044C-54 rough terrain forklift truck that had a wooden Gable on its forks when it tipped over onto an employee. The operator was not trained in the operation of this type of vehicle, nor was his performance evaluated in the workplace, and he was not certified as being trained and evaluated. ABATEMENT DOCUMENTATION REQUIRED
